SINGLE PROPRIETORSHIP

Basic Requirements

  • Bio-data with 2”X 2” picture (both husband and wife)
  • Valid ID’s with photo [any two (2) of the following for both husband and wife] — SSS/ GSIS (credit card type), PRC/ IBP, Senior Citizen, passport, Driver’s License w/ Official Receipt
  • Community Tax Certificate (both husband and wife)
  • Current Account Bank Statements (4 months latest)
  • Savings Account Passbooks showing account name, account number, balances (4 months latest)
  • Other Assets (certified True Copy of TCT and OR/CR of vehicles)
  • Marriage Contract

Additional Requirements for Single Proprietor

  • DTI Registration of Trade Name
  • Mayor’s Permit and Municipal License
  • List of five (5) suppliers with address, telephone number/s, contact person/s, goods or merchandise acquired, length of business relationship, terms of payment
  • Income Tax Return (ITR)
  • If with Special Power of Attorney (SPA), must be duly authenticated by the Consul.  Use of SPA is allowed only if one of the parties is out of the country.  Use of SPA is not allowed if the party is only in the province
